Blue Sky Support Services of Brainerd, MN is seeking an Direct Support Professional with creativity, compassion, and enthusiasm that can work full-time Mon-Friday, 8am-4pm, providing direct support services and/or job coaching to adults with disabilities. The primary purpose of the Direct Support Professional position is to ensure the safety and well-being of the individuals Blue Sky, Inc. provides services to, while implementing individual outcomes, documenting progress, and response to program services. Previous direct service care and/or job coaching experience preferred, and/or the ability to provide one-on-one care in a team-friendly setting. Fun and casual work environment. Job Coach:
Blue Sky, Inc. Provides individualized training and follow-up support services, which enhances stability and job retention for our participants in employment situations. We presently are seeking a part-time Job Coach that can provide one-on-one job coaching with adults with disabilities in the Brainerd area. Up to 20-25+ hours/per week, with strong potential of full-time soon thereafter. Responsibilities to include:

Provides necessary job shadowing support (on site job training or coaching), to ease transition of worker into community business site.

Assists participant in transferring generalized learned skills to a new site making adjustments that are necessary.

Assists participant in meeting production demands of the employment position during their training-in period.

Provides an open channel of communication with the employer, families, other team members, and placement specialist in regard to progress and performance.

Adapts to a number of work site environments involving a variety of tasks, duties, employer/employee relationships, new production rates (of different jobs), operations and procedures, etc.

Assists the employer in modification/restructuring of jobs for workers with disabilities, when necessary.

Arranges scheduled follow-up visits to provide job stabilization support services for employer and worker, as appropriate.

Completes time studies on supported employees in the community to monitor and establish commensurate wage.

Coordinates all case management activity and record keeping required by Blue Sky Inc. and government agencies (i.e., case notifications, wage and hour reports, etc.).

Provides participant performance data to Direct Support Supervisor on a timely basis.

Consults with Direct Support Supervisor to assist in determination of case closure, staffings, continued or auxiliary services suggested.

Assists participant in developing transportation independence.

Assists job candidate in transition to community employment through developing social skills, personal awareness, and understanding of each individual abilities as well as limitations.

Provide assistance and coverage at Blue Sky Inc. work sites, when needed.

Conducts oneself as a positive role model for participants in terms of appearance, socialization, conversation and teamwork.
